Appraisal or Proof of Authenticity based on an independent authenticator's physical inspection (required for Misrepresentation claims-must include contact name and number for authenticator)

Claim processing may be denied or delayed without required information. All of the required documents must be received by the Claims Administrator within 90 days of the end of the listing in order for your claim to be processed. Claims received after 90 days of the end of the listing will be denied. Any person who, knowingly and with intent to injure, defraud or deceive, files a statement of claim or an application containing false, incomplete, or misleading information may be guilty of a criminal offense and will be prosecuted to the fullest extent of the law.
